Multiple sclerosis and steroid drugs helpful in minimizing disease activityWhen a combination of steroid drugs and MS drug is administered to patients suffering from multiple sclerosis (MS), the level of disease activity can be reduced to a significant extent. This finding was revealed by a study presented as a part of the Late-breaking Science Program at the American Academy of Neurology’s 61st Annual Meeting in Seattle.

For the study, people with MS received the steroid drug methylprednisolone in monthly “pulses,” or three doses over three days, in addition to regular weekly treatment with the drug interferon beta-1a. The steroid drug has typically been used only to treat acute MS attacks, not as an ongoing treatment.

The study involved 341 people with relapsing-remitting MS. Half of the participants received both drugs; half received only the interferon drug plus a placebo. The participants were seen every three months during the three-year study for evaluation.

The participants had the disease for an average of three years and had not yet received a disease-modifying drug such as interferon.

Those who received both drugs had 38 percent fewer relapses, or times when the disease is active, than those receiving only the interferon drug. They also improved slightly on a test of MS disability, while the scores for the placebo group decreased slightly.

It was noted by study Mads Ravnborg, MD, of the Danish Multiple Sclerosis Research Center at Copenhagen University Hospital in Denmark that both drugs seem to benefit from a synergy when taken together with an aim to offer beneficial effect on the disease activity as against use of the MS drug in isolation.

Discovery of autoinflammatory syndrome among childrenResearchers from the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), part of the National Institutes of Health, and other institutions, have been able to identify a new autoinflammatory syndrome that affects children around the time of birth.

The syndrome is known as DIRA (deficiency of the interleukin-1 receptor antagonist).

The scientists identified nine patients from six families with DIRA in the Canadian province of Newfoundland, the Netherlands, Lebanon, and Puerto Rico. Those who were alive at the time of diagnosis – six in all – were treated with anakinra, a drug that is normally used for rheumatoid arthritis and is a synthetic form of human IL-1Ra. Although the patients were resistant to other medications such as steroids, most responded successfully and immediately to anakinra. “Our first patient had been unresponsive to several treatments, and his health care team had almost given up. But with anakinra, he was out of the hospital in 10 days and his symptoms resolved,” Dr. Goldbach-Mansky said.

Although the mutation that causes DIRA is rare, as many as 2.5 percent of the population of northwest Puerto Rico are carriers. Since DIRA is recessively inherited, these data suggest that it may be present in about 1 in 6,300 births in this population. Because the mutation was found in three independent Dutch families, newborn screening for DIRA in this population, as well as that of northwest Puerto Rico, may be warranted, Dr. Goldbach-Mansky said.

Children affected by this syndrome show a wide variety of severe and potentially severe symptoms including bone tissue swelling.

Millions of asthma sufferers use the inhalers also known as salbutamol or salbuterol to provide relief from wheeziness, as well as inhaled steroids to prevent the condition occuring. But the chemical byproduct in Ventolin inhalers may weaken the steroids anti-inflammatory effect.

The findings were revealed to a meeting of the American Academy of Allergy, Asthma and Immunology in San Francisco yesterday.

Dr Mark Aronica, of the Cleveland Clinic in Ohio, said: ‘The concern is that this chemical could build up in patients who take the drug frequently. ‘We need clinical studies to find out whether this is the case.’

This finding is expected to encourage clinical studies in the near term for eliminating the possible risk due to the presence of inhaler compound.

Signaling molecules like PTH have their effect in the body by interacting with specifically shaped proteins on the cell surfaces called receptors. PTH docks into its receptors, like a ship coming into port, which changes the shape of the dock such that biochemical signals are sent.

The authors of the current study observed that chondrocytes within injured and degenerating cartilage have more PTH type 1 receptors on their surfaces. This makes them especially sensitive to the PTH signal that prevents harmful chondrocyte maturation into bone in the joint cartilage. Thus, PTH therapy should increase the cartilage supply exactly where cartilage loss is causing disease.

Randy Rosier, M.D., Ph.D., professor within the Department of Orthopedics and Rehabilitation at the University ofRochester Medical Center , said that physicians are left with no answer when it comes to restoration of the cartilage in patients suffering from osteoarthritis.

“Our hypothesis is that just being afraid of being blown up all the time means you have high levels of steroids all the time,” Berger said, referring to PTSD among military personnel.

Low levels of glucocorticoids have anti-inflammatory effects in the brain, but high levels can trigger inflammatory mechanisms that damage nerve cells by activating an enzyme that causes oxidative stress. Even a single exposure to a high dose of glucocorticoids can be sufficient to damage nerve cells: A previous study showed synthetic glucocorticoid therapy to treat autoimmune disorders such as rheumatoid arthritis can induce mood disorders, including psychosis, and cognitive impairment known as “steroid dementia” in severe forms.

To determine the effects of prazosin, OHSU and PVAMC researchers, led by Altaf Darvesh, Ph.D., formerly of the OHSU Department of Psychiatry, administered a glucocorticoid called dexamethasone to rats, then measured the expression of a protein known as heat shock protein 70, or HSP70, that serves as a marker for neurotoxicity. Pretreatment with prazosin, an alpha-1 receptor antagonist, resulted in “significant” slowing of dexamethasone-induced expression in the cerebral cortex.

Berger further remarked that future research will be examining how steroids pose a threat to the brain and when prazosin should be administered to most effectively protect the brain against damage.x
